Before
preparing a Training Program Registration application, read the Training
Program Requirements carefully.

For Trainees. You are advised to read through these documents
before submitting an application for registration of your program, so
that you are well informed about the extent of the requirements before
you proceed further.
Acceptance
into a Residency Program or agreement
of an AVDC Diplomate to be your Alternate
Pathway Mentor
is the first step required.
Following
acceptance into a Residency Program or having reached agreement with
a Diplomate to be your Training Program Mentor, the next step is to
request log in access to the AVDC Document Management System (DMS).
Send an e-mail to the AVDC
Executive Secretary requesting DMS log-in access for registration
of your program, stating the name of your Training Program Supervisor
(Residency Program Director or Mentor) and the proposed Program Registration
start date.
Your
DMS UserName will be your FirstNameLastName (no space between first
and last names). You will receive confirmation of DMS registration and
notification of your temporary password by e-mail.
Following
receipt of the e-mail, read the Getting Started in DMS document (available
on the DMS login page) and then log in to DMS using your assigned UserName
and temporary password.
Complete
the personal information section, changing your password as you wish.
Note that some of the items in the Personal Information section, such
as the Status line, Supervisor name and Program Registration date, are
reserved for AVDC administrative use.
Now
you are ready to assemble and submit the Program Registration application.
An
application may be submitted up to 6 months prior to the proposed
program registration date.
The proposed registration date will not be approved by AVDC if the
application is received more than 30 days following
the proposed registration date.
The AVDC review process for the program registration process may extend beyond the proposed starting date of the training program. You may start to log cases and involve yourself in other training program activities following your proposed start date pending confirmation of registration of your program.
An
application consists of two parts. Review of the Application for Program Registration will not commence until items 1 and 2 have both been received by AVDC.
1. A completed Training
Program Registration form signed by the applicant
and supervisor, and the registration fee. These items are to be submitted
to the AVDC Executive
Secretary by mail. The registration fee is US$100, which is non-refundable.
2. An Application for Program Registration document submitted electronically
via DMS following review of the files by your program supervisor. Read the DMS User's Guide (available by a link on the Welcome screen after you have logged in to DMS). Click the Begin a New Document command on the right side of the Welcome screen, click Application for Program Registration as the Document Type, click Save and Next, then click Attach File or Attach Multiple Files on the Application for Program Registration screen. Follow the prompts to upload the files, then click the Submit this document.... check box in the yellow Current Document Status window on the right of the screen, and finally click Save Changes in the top command line.

following items are to be included in the Program Registration document:
For
a Residency Training Program
at an AVDC-approved Resideny Site:
A.
Applicant's curriculum vitae.
B.
Proposed arrangements for meeting the anesthesia, radiology, surgery
Specialist Training requirements.
For
an Alternate Pathway Training Program:
A.
Applicant's curriculum vitae.
B.
Proposed arrangements for meeting the anesthesia, radiology and
surgery Specialist
Training requirements.
C.
Proposed arrangements for meeting the Diplomate Visitation
Requirement.
D.
A list of the equipment and supplies available to you, indicating
that you are equipped to
commence the planned training program. See AVDC Initial
Equipment List.
E.
A list of books and journals available to you.
Additional
Notes:
You
may log cases and participate in other training program activities
from the proposed registration start date of the training program,
pending confirmation of registration of the program. Please note
that confirmation by AVDC of registration of the training program
and start date is required before training program activities will
be accepted for AVDC Credentials purposes.
AVDC
registration of a training program recognizes that the program is
acceptable to AVDC as described by the applicant and upon validation by
the program supervisor. Registration by AVDC is, of itself, not
an assurance of the quality of training, or that the applicant will
successfully complete the AVDC credentialing and examination requirements.
